About the role
Firms must be prequalified to provide professional services or construction services to Tectonic. Firms providing products or other services are not required to be prequalified.
Responsibilities
- Answer all questions and provide all requested information.
- Indicate N/A for questions not applicable and provide an explanation if necessary.
- Use supplemental information sheets for additional details that don't fit on the form.
- Clearly indicate the section of the questionnaire the supplemental information sheet references.
- Review the questionnaire for completeness before clicking the submit button.
Requirements
- Upload certificates verifying the status of being economically disadvantaged, women owned, veteran owned, etc., if applicable.
- Upload any judgments, arbitrations, liens, insurance claims, or lawsuits from the past 5 years.
- Indicate whether the firm has ever failed to complete a contract or had a contract terminated.
- Provide explanations for any affirmative responses to the criminal background information questions.
- Provide standard coverage limits for insurance policies including occurrence and aggregate limits.
- Indicate willingness to purchase additional insurance coverage if required by a contract.
- Have a written Health & Safety Program and site-specific plans.
- Conduct field safety inspections and document findings.
- Have an accident investigation and reporting program.
- Have a drug and alcohol screening program.
- Have a health and safety training policy and safe work practices.
- Train employees on driving, electrical, fall protection, hazard recognition, injury reporting, personal protective equipment, fire prevention, housekeeping, emergency response, first aid, safety meetings, radio frequency energy safety, competent tower climber and rescue training, confined space training, energy isolation and lockout/tagout, hazard communication, motorized equipment operator training, excavation and trenching, and fall arrest equipment.
