Responsibilities
- Conduct assessments and develop individualized service plans
- Connect participants to community resources and services
- Maintain accurate case documentation and ensure compliance
- Provide advocacy, crisis support, and skill-building activities
- Cookordination services with staff and community partners
- Monitor participant progress and update plans as needed
Requirements
- Education: Bachelor’s degree in Social Work, Human Services, Psychology, or related field
- Experience: 2 years of experience in case management, community support, or social services preferred
- Knowledge of community resources and public assistance programs
- Strong organizational, documentation, and time-management skills
- Ability to work independently and collaboratively within a team
- Strong communication and interpersonal skills
- Ability to apply professional judgment and maintain accountability
- Proficiency in case management systems, Microsoft Office, and data entry
Qualifications
- Valid North Carolina driver’s license and reliable transportation
- Ability to pass background checks and drug screening
- Availability to work flexible hours, including evenings and weekends
- Maintain strict confidentiality
- Demonstrate cultural competence and sensitivity
- Strong customer service and communication skills
- Commitment to continuous quality improvement

Pay
$45,200 – $48,000 annually
Schedule
Monday – Friday, 8:00 AM – 5:00 PM (some evenings and weekends required)
