Responsibilities

  • Conduct assessments and develop individualized service plans
  • Connect participants to community resources and services
  • Maintain accurate case documentation and ensure compliance
  • Provide advocacy, crisis support, and skill-building activities
  • Cookordination services with staff and community partners
  • Monitor participant progress and update plans as needed

Requirements

  • Education: Bachelor’s degree in Social Work, Human Services, Psychology, or related field
  • Experience: 2 years of experience in case management, community support, or social services preferred
  • Knowledge of community resources and public assistance programs
  • Strong organizational, documentation, and time-management skills
  • Ability to work independently and collaboratively within a team
  • Strong communication and interpersonal skills
  • Ability to apply professional judgment and maintain accountability
  • Proficiency in case management systems, Microsoft Office, and data entry

Qualifications

  • Valid North Carolina driver’s license and reliable transportation
  • Ability to pass background checks and drug screening
  • Availability to work flexible hours, including evenings and weekends
  • Maintain strict confidentiality
  • Demonstrate cultural competence and sensitivity
  • Strong customer service and communication skills
  • Commitment to continuous quality improvement

Pay

$45,200 – $48,000 annually

Schedule

Monday – Friday, 8:00 AM – 5:00 PM (some evenings and weekends required)
