Community Services - Program Manager-HRSN

Responsibilities

• Operate within the philosophies of Harm Reduction, Motivational Interviewing, and the Housing First Model to assist individuals in meeting their needs and fostering empowerment.
• Oversee the Community Supports navigation services, including hiring, supervising, mentoring, and evaluating navigators.
• Provide oversight of caseload management and staff assignments of individuals served.
• Attend insurance provider meetings and develop collaborative relationships with those providers.
• Assess healthcare needs and assist in the coordination and referral for healthcare services.
• Maintain accountability for the billing of each navigator and ensure productivity requirements are met.
• Ensure timely completion of all Assessments and Support Plans according to prescribed timelines and specifications.
• Work as a liaison and advocate with employment programs, employers, therapists, doctors, and social security staff to assist individuals served as needed.

Requirements

• A High School Diploma, GED, or equivalent.
• A valid Driver’s License with an acceptable driving record and a reliable vehicle with proof of insurance.
• A minimum of two years’ professional or lived experience with housing supports and/or behavioral health.
• Previous supervisory experience in a professional setting.
