Community Services IDD Provider Specialist

Responsibilities

• Completes assessments and identifies needs
• Teaches Supported Home Living/Community Supports skills classes
• Teaches Community First Choice (CFC) Personal Assistant Services (PAS) and Habilitation (HAB) services
• Provides transportation
• Documents services provided

For Individuals Referred for Community Employment

• Develops employment opportunities with local businesses for individuals in waiver and general revenue programs only
• Uses structured intervention techniques to help individuals learn essential soft and hard skills of the job and/or arrange transportation
• Works with individuals and employers to establish support services, accommodations, compensatory techniques, and training
• Monitors individual's performance to ensure improvement
• Gradually reduces time spent with the individual at the job site as the individual becomes more adjusted and independent

Qualifications

• High School diploma or GED plus six months of work experience serving individuals with developmental disabilities
• Valid Texas driver’s license and acceptable driving record, along with personal automobile liability insurance as required by the state of Texas

Preferred Qualifications:

• High School diploma or GED plus two years of work experience serving individuals with developmental disabilities
• Valid Texas driver’s license and acceptable driving record, along with personal automobile liability insurance as required by the state of Texas
