Description and Responsibilities

The Enrollment and Care Coordination Specialist will screen individuals for Open Sky’s Community Supports Program (CSP-HI), Housing, Nutrition Program, and Navigation and Transitional Goods eligibility. They will complete enrollment for individuals in the program, including orientation to services, service agreements, consents, and data entry into the electronic health record software for each member. They also provide training, monitoring, feedback, and corrective action to housing specialists and assign and track housing specialists caseloads in collaboration with the team lead, as needed.

  • Outreach to community providers to develop relationships and develop referral pathways.
  • Verify insurance prior to enrollment and monitor insurance eligibility for each member.
  • Obtain authorizations from the insurance provider for each member.
  • Track authorization expiration and obtain new authorizations.
  • Participate in live review meetings with insurance company representatives for each member.

About the Role

Open Sky’s Community Supports Program is a Mass Health funded navigator service that provides case management and care coordination to individuals in the community. Individuals served in the program must have a mental health disorder or co-occurring disorder and be considered at high risk for admission to a 24-hour treatment setting. Other services include the Health Related Social Needs (HRSN) program, which works in collaboration with MassHealth to address the immediate daily necessities that arise from the inequities caused by the social determinants of health, such as lack of access to basic resources including stable housing, a toxin-free environment, healthy food, heating and Internet access, transportation, healthcare, safety, education, employment, and social connection.

Qualifications

  • A High School Diploma, GED or equivalent, required.
  • Minimum of two years’ experience with housing and behavioral health population, required.
  • A valid driver’s license and acceptable driving history, required.
  • Ability to collaborate with a multi-disciplinary team, possess excellent written and verbal communication skills with attention to detail, required.
  • Experience with electronic health records, billing and/or referral management are strongly preferred.

Pay and Schedule

Base Rate USD $25.55/Hr.
