Responsibilities
- Screens individuals for Open Sky's Community Supports Program (CSP-HI), housing, nutrition program, navigation, and transitional goods eligibility.
- Completes enrollment for individuals in the program, including orientation to services, service agreements, consents, and data entry into the electronic health record software.
- Provides training, monitoring, feedback, and corrective action to housing specialists.
- Assigns and tracks housing specialists caseloads in collaboration with the team lead, as needed.
- Outreach to community providers to develop relationships and establish referral pathways.
- Verifies insurance prior to enrollment and monitors insurance eligibility for each member.
- Obtains authorizations from the insurance provider for each member.
- Tracks authorization expiration and obtains new authorizations.
- Participates in live review meetings with insurance company representatives for each member.
Requirements
- A High School Diploma, GED, or equivalent, is required.
- Minimum of two years' experience with housing and behavioral health populations is required.
- A valid driver’s license and acceptable driving history are required.
- The ability to collaborate with a multi-disciplinary team, possess excellent written and verbal communication skills with attention to detail, is required.
- Experience with electronic health records, billing, and/or referral management is strongly preferred.
