Community Relations Liaison

FUNCTIONS & RESPONSIBILITIES

• Work in conjunction with the admission team.
• Meet with facility staff and other community members to establish an organizational presence within assigned territory.
• Aid the Admission Team with obtaining any outstanding admission documents and patient financial information, as needed.
• Go into the field and establish presence of our organization to our service members.
• Respond to referrals for evaluation and/or admission to the organization’s program.
• Verify and complete all admission and financial information.
• Explain the program to patients, family members, and other referral sources.
• Collaborate with the admission team for referrals, admissions, and care coordination of patient and family needs.
• Identify key referral sources and assist the admission counselors to develop a marketing plan for the program.
• Attend and participate in in-services, staff meetings, and case conferences.
• Complete all documentation in an accurate and legible manner.
• Comply with company policies and procedures, laws, and regulations, and standards of practice.
• All other tasks and duties deemed necessary and appropriate.
