Parker At Your Door, a medical house calls program serving the greater New York Metro region, is seeking a Primary Care Physician (MD/DO) to provide quality primary medical care to patients in the comfort of their homes. Now conducting visits in Suffolk County—full-time, part-time, and per-visit opportunities available.

As a Primary Care Provider, you will assess members with multiple comorbidities residing in the community. This position involves performing comprehensive evaluations of patients and providing medical management. About Parker
The Parker Jewish Institute for Health Care and Rehabilitation, located on the Queens-Nassau County border in New Hyde Park, New York, is a non-profit health care facility. It offers inpatient programs such as sub-acute/short-term rehabilitation, long-term care, and nursing home care, as well as community health services including certified home health care and a comprehensive community hospice program. The hospice program serves terminally ill patients in their own homes or in nursing facilities, including Parker's nursing home.
Responsibilities
- Examine patients and order appropriate diagnostic workups
- Assess members with acute changes in condition in a timely manner
- Maintain complete and accurate medical records
- Consult with other members of the interdisciplinary team to create appropriate and viable care plans
- Educate patients on topics such as disease process, medication, and compliance
- Provide medical management of patients
Requirements
- Licensure and current registration to practice as a Physician in New York State
- Strong communication and follow-up skills
- Proficiency in Microsoft Office, including Excel, data entry, documentation, and report generation
- Prior experience with the geriatric population and home care strongly preferred
- Knowledge of medical documentation
