Community Partners Supervisor
About the Role
The Community Partners Supervisor develops and manages the Community Partners team to individualize community-based activities for participants based on their beliefs, interests, preferences, and support needs. This role maximizes participant inclusion in the community by leveraging networking, support resources, and natural support networks through assistive and adaptive technologies. The supervisor establishes professional and collaborative relationships with internal and external stakeholders to maintain consistent, timely, and open communication.
Responsibilities
- Collaborate with agency leadership to provide coaching, performance improvement plans, and support to team members, emphasizing equity, care, and compassion.
- Participate in the hiring and onboarding process, including submission of Peer Training schedules to HR.
- Ensure team members understand their roles, expectations, policies, and procedures while equipping them with tools and techniques to optimize resources and provide quality supports.
- Assign and manage routes and daily activities, stepping in when necessary to ensure adequate supports are maintained.
- Facilitate department functions, including monthly meetings, check-ins, status changes, transitions, trainings, and incident reports.
- Conduct a minimum of one site visit per week.
- Attend monthly leadership meetings.
- Review GPS alerts from Transportation and schedule team members for corrective action when needed.
- Maintain confidential and accurate records for team members (e.g., attendance, goals completion, performance evaluations) and complete required documents in a timely manner.
- Review and approve weekly timesheets and mileage forms.
- Attend individual Pre-PCP meetings as necessary, participate in PCP signing meetings, and ensure staff is trained on new PCPs within 20 days after the meeting; submit signatures to the Case Coordinator.
- Undertake other duties as assigned, including special projects and cross-program development; respond promptly to QE reviews within deadlines.
- Ensure the supported person’s plan is conducted as outlined in written plans, programs, schedules, and other documentation.
- Review PMOF books to ensure availability of current PMOF and/or scripts.
- Cultivate and maintain mutually beneficial relationships with community members and organizations to achieve service and participant goals.
- Monitor crisis situations, collaborate with authorized staff and the supported person to adjust supports and the environment while adhering to policies and regulations.
- Develop, maintain, and effectively use information management systems to evaluate service and program outcomes and standards.
- Communicate with the Assistant Director regarding urgent care visits, hospitalizations, vehicle accidents, major staffing issues, or any police, fire, or ambulance involvement.
- Review goal documentation to ensure support of billing.
- Monitor the accuracy and timeliness of attendance tracking and recording.
- Review monthly expenses and reconcile monthly petty cash.
- Collaborate with the Finance Department to ensure compliance with agency policies and budgetary goals.
- Plan and implement day-to-day operations, activity schedules, and short-term objectives with optimal resource utilization and adherence to budget.
Requirements
- High School Diploma required; Associate’s or Bachelor’s in Human Services preferred.
- DSP I certification or ability to complete within the first 90 days.
- Two years of supervisory experience.
- Three years of experience in the human services field.
- Fluent in Microsoft Office Suite (Word, PowerPoint, Excel, Outlook).

Additional Requirements
- Access to reliable transportation.
- Active valid driver’s license with no more than two points and a clean driving record.
- Proof of current automobile insurance.
- Maintain all required trainings under DDA guidelines and agency policies.
- Flexibility in scheduling to meet agency and program needs.
- Ability to obtain a DOT Medical Card (Non-CDL) within 30 days of employment.
