What You’ll Do
- Aid patients with completing applications, registration forms, and accessing community resources
- Help patients connect with transportation services and provide appointment reminders
- Support seamless care coordination from initial contact through case closure
- Offer general office and clerical support to the outpatient care management team
- Guide patients in navigating the healthcare system with confidence and clarity
- Contribute to population health goals through outreach and patient engagement
What You Bring
- Education: High school diploma or equivalent required; Associate Degree preferred
- Experience: Clerical experience in a healthcare setting preferred
- Familiarity with Microsoft Office, fax systems, and electronic medical records preferred
- Skills & Abilities: Strong communication and computer skills; Knowledge of community resources (preferred); Basic understanding of medical terminology; Ability to prioritize tasks, stay organized, and manage multiple responsibilities
