What You’ll Do

  • Aid patients with completing applications, registration forms, and accessing community resources
  • Help patients connect with transportation services and provide appointment reminders
  • Support seamless care coordination from initial contact through case closure
  • Offer general office and clerical support to the outpatient care management team
  • Guide patients in navigating the healthcare system with confidence and clarity
  • Contribute to population health goals through outreach and patient engagement

What You Bring

  • Education: High school diploma or equivalent required; Associate Degree preferred
  • Experience: Clerical experience in a healthcare setting preferred
  • Familiarity with Microsoft Office, fax systems, and electronic medical records preferred
  • Skills & Abilities: Strong communication and computer skills; Knowledge of community resources (preferred); Basic understanding of medical terminology; Ability to prioritize tasks, stay organized, and manage multiple responsibilities
