About the Role
The Community Nurse Navigator collaborates with community-based organizations, faith-based organizations, worksites, and other healthcare providers to promote population health and wellness. This role works with the Care Management Team to formulate and communicate transition/discharge plans that efficiently utilize healthcare services in a safe and cost-effective manner. Additionally, the Community Nurse Navigator provides education and care coordination for specific “at-risk” patient populations to reduce barriers to healthcare.
Responsibilities
- Collaborate with community-based organizations, faith-based organizations, worksites, and healthcare providers to promote population health and wellness.
- Work with the Care Management Team to develop and communicate transition/discharge plans that ensure safe and cost-effective use of healthcare services.
- Provide education and care coordination for “at-risk” patient populations to reduce barriers to healthcare access.
Requirements
- Currently licensed as a Registered Nurse in the State of Texas.
- Bachelor of Science in Nursing is preferred.
- At least two years of acute care experience.
- Care Management or discharge planning experience preferred.
- Advanced knowledge of community resources.
Qualifications
- Disease and/or quality management experience or community health experience preferred.
Working Conditions
- Must be able to move about the hospital without major constraints.
- Universal precautions are maintained at all times.
- Must be able to work long hours on occasion and work well under tight deadlines.
