About the role

This position is responsible for providing community relationship development with health, human, and social service community partners to best encompass the community-centric needs of the main site. The position serves as a resource representative for citizens in each district area to help navigate needs such as food insecurity, homelessness and emergency housing, utility and rental assistance, and other targeted social needs. Additionally, this position provides key resources and assistance at Community Resource Centers for current health partners and the community area served onsite.

Essential Duties

  • Identifying and collaborating with community partners onsite and within the community at large through active communication, decision-making, and problem-solving
  • Serving residents, community organizations, and stakeholders onsite and through One Stop for Help programs including warming stations, encampment, and vulnerable populations such as veterans and seniors
  • Coordinating services with relevant community resources and other service providers and agencies to ensure a holistic approach to resident need and outreach
  • Maintaining detailed, accurate and up-to-date records, case notes, progress updates, key service activities and documentation of services provided
  • Monitoring participant progress and following up on referrals through virtual or in-person outreach to identify resource needs, connecting residents to appropriate services, and evaluating the effectiveness of interventions to ensure needs are met
  • Providing and collaborating with key resource and assistance necessary through mobile senior expansion and accessing program and radius communities
  • Communicating and advocating with internal and external partners to identify and resolve community needs
  • Providing education and outreach through individual, group, and community efforts to raise awareness and engagement
  • Supporting part-time Seasonal Warming Station attendants and assist with rotating station support, including assistance with delivery and offloading supplies (e.g., cots, water, snacks, cot covers, blankets) using county vehicles
  • Collecting intake files and engaging with residents on-site (Warming Station/Encampment Outreach) to ensure needs are met and documentation is complete
  • Conducting site visits to encampments where initial contact has been established by a Police Officer, Community Representative, or local residents and safely engaging with individuals or groups to share resources and offer support (Encampment Outreach)

Public Speaking Duties

  • Educating and providing information about programs, services, and opportunities to the public at community events, resource and job fairs, and other forums, as needed
  • Developing and engaging in relationship building through outreach with groups, agencies, organizations, and resident-related interactions

Other Job Requirements

  • Participating in a rotational on-call schedule to support program operations and respond to time-sensitive resident or operational needs outside of regular business hours, including evenings, weekends, holidays, and periods of inclement weather or emergency operations (employees assigned to on-call rotation are expected to remain accessible and respond within established departmental procedures and response-time expectations)
  • Upholding ethical standards, cultural competency, and professional boundaries to provide excellent customer service and maintain sensitive interactions
  • Have knowledge of case management principles; applicable laws, rules, and regulations; counseling principles and practices; available resources to clients; and screening and assessment principles and practices

Minimum Qualifications

  • Bachelor's degree in social work or related field
  • One year of experience in case management
  • Or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job such as those listed above
  • Valid driver's license
  • CPR (within six months of employment)
  • First Aid (within six months of employment)

Preferred Qualifications

  • Bachelor's degree in social services, sociology, public health, human services, psychology, gerontology or related field
  • One year of experience in case management principles, applicable laws, rules, and regulations
