Responsibilities

  • Provide evidence-based nursing care within full scope of LPN practice
  • Obtain and update medical histories and medical information on the EMR
  • Schedule and coordinate referrals for tests, follow up to other disciplines and utilize community resources as needed
  • Able to assist with medication administration as assigned
  • Obtain and document weight, height, girth, and vital signs
  • Maintain therapeutic nurse-person served relationships
  • Provide updates on persons served medical status and needs to mental health treatment team and coordinate care with team
  • Provide health teaching, addressing topics such as healthy lifestyle, risk reduction behaviors, and preventative self-care
  • Participate in quality improvement activities in collaboration with the PACT Registered Nurse
  • Participate in ongoing educational activities related to learning needs and professional issues
  • Serve as a person served advocate and assist him/her to advocate for themselves
  • Administer vaccines and IM injections deemed necessary by provider during the visit, and may draw blood samples or collect urine samples for lab testing as necessary
  • Actively seek out supervision from own supervisor at least monthly, and participate in group nursing practice supervision as assigned

Requirements

  • High school diploma or equivalent, must have graduated from an accredited nursing training program and possess and maintain licensure, in good standing, in the State of NJ as an LPN
  • Current CPR Certificate required (BLS)
  • Immunization course
  • Medication course
  • IM Course
  • Driver’s License, Vehicle Valid driver’s license required
  • No more than one moving violation within the past 12 months
  • Vehicle required

Qualifications

  • Education/Certifications: High school diploma or equivalent, must have graduated from an accredited nursing training program and possess and maintain licensure, in good standing, in the State of NJ as an LPN. Current CPR Certificate required (BLS).
  • Experience: Immunization course, Medication course, IM Course
  • Skills: Driver’s License, Vehicle Valid driver’s license required, No more than one moving violation within the past 12 months, Vehicle required
