Community Liaison

About the role

As the face of hospice care in the San Jose area, you will champion our commitment to dignity, comfort, and human connection. You'll actively promote specialized hospice services by developing and nurturing trust-based relationships with physicians, referral sources, and healthcare partners. Your work will drive growth and increase awareness of the compassionate care families rely on during critical times. This role offers a Monday–Friday, 8:00 am to 4:30 pm schedule and the opportunity to grow professionally in a collaborative environment that values your contributions.

Responsibilities

• Build and maintain strong relationships with key decision-makers like physicians, hospital discharge planners, social workers, and skilled nursing staff to foster collaboration and trust.
• Act as a passionate ambassador for hospice care, sharing our mission and services through consistent outreach and education.
• Increase referral growth by regularly connecting with healthcare professionals and community agencies in your territory.
• Support families by leading sensitive conversations about hospice care, explaining our philosophy clearly and compassionately.
• Help position hospice as the provider of choice throughout San Jose and surrounding areas to exceed growth and census goals.

Qualifications

• Minimum two years of sales or marketing experience in hospice or palliative care.
• Proven success in building and sustaining professional relationships with healthcare providers and community partners.
• Ability to communicate empathically and effectively during delicate family interactions.
• Reliable transportation, a valid driver's license, and auto insurance.
• Bachelor's degree preferred; equivalent experience considered.

Schedule

Monday–Friday, 8:00 am to 4:30 pm.

Pay

Highly competitive base salary with uncapped monthly commission.
