About the role

ServiceSource is an organization dedicated to driving innovation, embracing change, and strengthening communities. This position involves creating, leading, and supporting assigned participants in various areas such as academic, social, life and leisure skills, and recreational activities.

Responsibilities

  • Provide active engagement and support for all individuals assigned, including community integration/inclusion, training, leisure, communication, mobility, behavior, personal care, bus loading/unloading, medication, safety and risk management, etc.
  • Implement and maintain Person Centered Plans (PCP) including social, community inclusion, communication, behavior, and skill development.
  • Ensure compliance with State and Federal regulatory guidelines, including Medicaid, Licensure, and CARF as well as other funding sources and company policy/procedure.
  • Maintain accurate participant records, daily notes, quarterly reports, etc. per Medicaid, Licensure and CARF standards.
  • Communicate with family members, guardians, authorized representatives, residential providers, and Support Coordinators regarding participant's illness, incidents, progress, etc.
  • Complete incident reports as required and report any allegations of abuse, neglect, and exploitation according to ServiceSource policy/procedures.
  • Participate in department, participant, and staff committee meetings and provide input for the PCP process.
  • Participate in trainings as required by funding sources and the corporation.
  • Perform related duties and responsibilities as assigned/requested.

Qualifications

  • A High School diploma (or GED) required; one (1) year of experience working with adults with developmental disabilities or the related field preferred.
  • A combination of college level courses in the human services field and experience working with adults with developmental disabilities or a bachelor’s degree in the human services field and experience working with individuals with developmental disabilities.

Additional Duties and Responsibilities

  • Maintain participant records, daily notes, quarterly reports, etc. per Medicaid, Licensure and CARF standards.
  • Adhere to health and safety regulations.
  • Utilize appropriate behavior intervention techniques, proper lifting, feeding, and transferring techniques as written.
  • Complete and pass the Orientation to Development Disabilities (ODD) test and competencies.
  • Participate in department, participant, and staff committee meetings.
  • Participate in trainings as required by funding sources and the corporation.
  • Perform related duties and responsibilities as assigned/requested.

Physical Demands

  • Stand, reach with hands and arms, talk or hear.
  • Occasionally walk and sit.
  • Lift or move products and supplies up to 50 pounds.
