About the role

Create, lead, and support assigned participants in their daily programming in the areas of academic, social, life, and leisure skills as well as recreational activities. Assist in the development and implementation of Person-Centered Individual Service Plans (ISP) including social, community inclusion, communication, behavior, and skill development. Ensure compliance with all State and Federal regulatory guidelines, including Medicaid, Licensure, CARF, and other funding sources and company policy/procedure.

Responsibilities

  • Provide active engagement and support for all individuals assigned, including community integration/inclusion, training, leisure, communication, mobility, behavior, personal care (feeding, etc.), bus loading/unloading, medication, safety, and risk management.
  • Provide input and implement Person-Centered Plans (PCP) outcomes, support activities, and protocols to ensure a safe and healthy environment, proper documentation, and progress.
  • Provide and maintain accurate information and documentation for the PCP, quarterly reports, and daily notes.
  • Maintain a safe, clean, and orderly environment; immediately report any unsafe conditions, medical concerns, etc., to the Program Manager (or designee).
  • Follow established written protocols and plans; report any allegations of abuse, neglect, and exploitation according to ServiceSource policy/procedures.
  • Complete incident reports as required.
  • Ensure and maintain accurate records and documentation per the PCP, regulatory, and company standards.
  • Inform the Program Manager (or designee) of missing or inaccurate information immediately.
  • Communicate with family members, guardians, authorized representatives, residential providers, and Support Coordinators regarding participant’s illness, incidents, progress, etc., at the direction of the Program Manager (or designee).
  • Maintain participant records, daily notes, and quarterly reports per Medicaid, Licensure, and CARF standards.
  • Adhere to health and safety regulations.
  • Participate in the PCP process (plan, quarterlies, daily notes) and work with the team in the implementation, monitoring, and review of the individual’s written plan.
  • Utilize appropriate behavior intervention techniques, proper lifting, feeding, and transferring techniques as written.
  • Complete and pass the Orientation to Developmental Disabilities (ODD) test and competencies.
  • Participate in department, participant, and staff committee meetings.
  • Participate in trainings as required by funding sources and the corporation.

Requirements

  • A High School diploma (or GED) required; plus, one (1) year of experience working with adults with developmental disabilities or a related field.
  • Preferred: High School diploma (or GED) plus a combination of college-level courses in the human services field and experience working with adults with developmental disabilities, or a bachelor’s degree in the human services field and experience working with individuals with developmental disabilities.

  • Knowledge of communication and its resources.
  • Physical ability to support individuals with personal care (feeding, toileting, transferring, etc.).
  • Employees who drive participants in the community must possess a current and valid driver’s license and be eligible and covered under ServiceSource’s driver’s insurance; must pass the Online Driving training if eligible to drive.
  • Ability to pass a criminal background check and drug screening as required.
  • Working knowledge of Microsoft Office software and other basic computer programs.

Physical Demands

  • Regularly required to stand; reach with hands and arms; and talk or hear.
  • Occasionally required to walk and sit.
  • May occasionally lift or move products and supplies, up to 50 pounds.
  • Specific vision abilities required include close vision, distance vision, depth perception, and ability to adjust focus.
  • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Work Environment

The noise level in the work environment is usually quiet.

Pay

$18/hour
