Community Health Worker- Joplin

About the role

A community health worker is a front-line public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

Responsibilities

• Serves as a liaison/link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
• Works collaboratively with other healthcare providers and community partners to enhance service delivery.
• Provides education and resources to community members on health topics relevant to their needs.
• Assists with outreach efforts to identify and address unmet health needs in the community.
• Maintains accurate records and documentation in an electronic medical record system.

Requirements

• High School Diploma or equivalent
• 1-3 years CHW experience or equivalent
• Certification/Registration from Mercy provided Community Health Worker training within 6-12 months
• Basic Life Support certification
• Proficiency with Microsoft Applications
• Access to reliable transportation

• Strong relationship and collaboration skills
• Excellent communication skills
• Ability to work independently
• Compassion for poor and underserved members of the community
