Community Health Worker- Joplin
About the role
A community health worker is a front-line public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
Responsibilities
- Serves as a liaison/link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
- Works collaboratively with other healthcare providers and community partners to enhance service delivery.
- Provides education and resources to community members on health topics relevant to their needs.
- Assists with outreach efforts to identify and address unmet health needs in the community.
- Maintains accurate records and documentation in an electronic medical record system.
Requirements
- High School Diploma or equivalent
- 1-3 years CHW experience or equivalent
- Certification/Registration from Mercy provided Community Health Worker training within 6-12 months
- Basic Life Support certification
- Proficiency with Microsoft Applications
- Access to reliable transportation

- Strong relationship and collaboration skills
- Excellent communication skills
- Ability to work independently
- Compassion for poor and underserved members of the community
