Community Health Representative
Join Chapa-De's Care Coordination team as a Community Health Representative, where you'll help connect patients and families with the care, education, resources, and support they need. You'll build trusted relationships within the community, help patients navigate the health care system, coordinate services and referrals, and serve as an important link between patients, families, community partners, and health care providers.
Responsibilities
- Build trusting relationships with patients and families while providing outreach, education, support, advocacy, and care navigation.
- Help patients identify health and social service needs, connect with community resources, and coordinate in-house and outside referrals.
- Support patients in understanding and navigating the health care system, including medical and dental insurance enrollment, transportation resources, appointment reminders, and follow-up.
- Collaborate with health care providers, clinical teams, community agencies, and other Chapa-De departments to support coordinated, culturally appropriate care.
Requirements
- High school diploma, GED, or equivalent.
- Excellent verbal, written, and interpersonal communication skills with the ability to maintain a high level of confidentiality.
- Proficiency with accurate data entry in an Electronic Health Record system and Microsoft Office applications, including Word, Excel, and Teams.
- Experience working with Native American populations, along with sensitivity to and familiarity with Native American cultures and customs.
- Ability to establish and maintain effective working relationships, coordinate activities across teams and community organizations, and work independently in a flexible, self-directed manner.
- Current and in-good-standing driver's license, vehicle insurance, and DMV printout.
Pay
Hourly Pay: $27.35-$28.35
Schedule
Monday–Friday, 8:00 AM – 5:00 PM. Flexibility to work at our Rocklin clinic 1–2 days per week and extended-hour shifts as needed.
