Community Health Representative

About the Role

The Community Health Representative promotes Family HealthCare Network services through outreach activities, coordination of community events, and participation in relevant grant initiatives and special programs.

Responsibilities

• Carry out responsibilities in community outreach, enrollment assistance, coordination of FHCN community events, grants, and programs.
• Achieve departmental goals and metrics for assigned functional areas.
• Communicate feedback to the supervisor regarding workflows and policies.
• Serve as a subject matter expert in assigned functional areas.
• Perform high-level administrative functions to comply with regulations and reporting requirements.
• Execute outreach activities, participate in community events, and provide health screenings.
• Lead community focus groups and surveys.
• Conduct presentations and educational workshops to raise awareness of network services and programs.
• Maintain relationships with strategic partners.
• Act as a liaison between FHCN and patients, providing enrollment services and referrals.
• Conduct intake and screenings to determine eligibility for various programs.
• Educate consumers on health coverage benefits and rights.
• Provide ongoing case management to ensure healthcare coverage retention and utilization of FHCN services.
• Implement programs and grant curriculums at sites and in community settings.
• Manage day-to-day operations at food pantries, including inventory and product storage.
• Ensure compliance with program or grant-related training and certifications.

Qualifications

• Effective June 1, 2026, all individuals hired into the role must possess work-related skills beyond high school completion, including formal written and verbal communication skills, computational and computer skills, mathematical, and technical skills.
• Proof of completion of high school with a minimum GPA of 2.5 or completion of GED with a minimum score of 162.5.
• Health care-related knowledge, often acquired through completion of a trade school, para-professional, or certificate-type program.
• Minimum of two years of experience in community outreach, educational activities, and customer service.

Skills

• Proficiency in Microsoft Word, Excel, and PowerPoint.
• Ability to compile information in effective written form and convey technical information to non-technical audiences.

Requirements

• Valid CA driver’s license and car insurance.
• Certification as a Certified Enrollment Counselor (for Covered California education and enrollments).
• Food handlers certification (for staff supporting food programs).

Physical Demands

The employee is regularly required to sit and use repetitive hand movements. Frequently required to stand or walk, and occasionally stoop, reach, squat, climb, lift, and/or move up to 30 pounds. Must work in various environmental elements and travel between sites.

Min Hourly Rate: $21.56
Max Hourly Rate: $30.86
