COMMUNITY HEALTH ADVOCATE

Responsibilities

• Work collaboratively with clients and their families to provide supportive services and coordinated care.
• Conduct monthly home visits to assess needs, provide support, and ensure continuity of care.
• Complete health risk assessments to identify individuals who may benefit from supportive services related to birth equity, social determinants of health, and care coordination.
• Connect clients to appropriate community resources and enhanced social support, including childbirth education classes, workshops, and other perinatal services.
• Aid clients in developing individualized birth plans and postpartum care plans.
• Facilitate access to parenting education and support services, including: Parenting classes or workshops, Doula support services, Peer support groups, and clinical support groups such as postpartum support groups, Breast education and support.
• Deliver patient engagement and educational programming, including printed and electronic materials, for individuals of childbearing age.
• Promote healthy behaviors and reduce risk factors that may negatively impact birth outcomes, including preconception health education.
• Assist, as needed, with coordination of program activities, community events, and client referrals, in collaboration with staff and community partners.

Qualifications

• HS diploma/GED required
• Bachelor’s degree preferred in: Social Work, Psychology, Education, Rehabilitation, OT, Counseling, Community Mental Health, Sociology, etc.
• 2-4 years of related human services experience required in providing direct services to the underserved community of services essential to successfully living in the community, Peer with lived experience.
• Strong interpersonal and communication skills.
• This role requires flexibility to support community events, including occasional weekend work and some late-evening hours as needed.
• Bilingual in Spanish is required.
• Must be willing and able to conduct home visits at clients’ residences and travel to multiple locations throughout Nassau County using reliable and safe transportation.
• A valid driver’s license is required.
