Community Health Coordinator- Parkland Health Center
About the role
Parkland Health Center has a new, grant-funded position created through the Rural Health Transformation Grant and represents the first step in advancing regional healthcare transformation efforts. The role will support one of the state's hubs and help lead initiatives across three regional hubs and counties, working with stakeholders to identify priorities and drive key community health improvement efforts.
Responsibilities
- Facilitate and coordinate transformation work, partnering with community organizations, behavioral health providers, ambulance districts, public health agencies, and other healthcare stakeholders.
- Develop and maintain a network of community resource centers, including development and implementation of curriculum, services and resource development for all locations.
- Convene strategic partners and develop collaborations within the community to improve program relations and expand service locations.
- Conduct ongoing needs assessment of surrounding community/neighborhoods to determine the most relevant topics for health education in the community resource centers.
- Assist in planning, coordinating and implementing family connection meetings in the community.
- Facilitate recruitment and retention of participants in all program offerings.
- Conduct regular resource needs assessments within the communities we serve.
Requirements
- Demonstrate professionalism, excellent communication skills, community-focused leadership, and experience or interest in public health and community collaboration.
Qualifications
- Education: Associate's Degree
- Preferred Requirements: Bachelor's Degree
- Experience: 2-5 years supervisor experience
