About the Role
Provides person-centered counseling services to Veterans enrolled in the Veterans Directed Care (VDC) program and supports the care coordination department with other clients as needed. The Community Health Coordinator educates Veterans on the VDC program, acts as an advocate and coach, identifies their needs, and develops a Spending Plan to meet those needs. The role involves assessing, identifying, coordinating, and monitoring supports and services on an ongoing basis, as well as supporting the Veteran in the creation and management of their individualized Spending Plan. The Community Health Coordinator monitors all VDC spending alongside the Veteran, the Financial Management Service (FMS) provider, and the VAMC.
Responsibilities
- Conduct home visits (50% including travel time) for initial assessments, spending plan development, assisting Veterans with becoming employers, facilitating new hire appointments, and performing required monthly, quarterly, semi-annual, and annual contacts and reassessments.
- Provide training and remediation related to employer responsibilities for Veterans.
- Follow up on referrals and service implementations for clients in an office setting (50%).
- Maintain, update, and monitor clients' confidential information, services, and file notes in the database program.
- Perform fiscal duties under the Veterans Directed Care program, including maintaining accurate records of expenditures, generating monthly reports, and handling satellite office administration as assigned.
- Provide liaison services between agency programs and potential clients.
- Perform data entry in the agency client tracking program.
- Maintain strict confidentiality concerning all client and program operations information.
- Demonstrate and maintain good work ethics.
- Stay current with professional knowledge, community resources, and funding opportunities for seniors and disabled adults.
- Maintain travel schedules and related expense reports.
- Organize and prioritize schedules and duties independently.
- Maintain ongoing relationships with professionals, medical providers, and contractors on behalf of the agency.
- Work in and away from the office, including attending community events as assigned.
Requirements
- Knowledge of HIPAA regulations and general Medicaid & Medicare eligibility criteria.
- Proficiency with computers and Microsoft Office programs (e.g., word processing, spreadsheet, PowerPoint, email).
- Familiarity with Appalachian Agency for Senior Citizens policies and procedures.
- Ability to work independently, advocate for clients, and collaborate with families and community partners.
Qualifications
- Bachelor’s degree in social work, human services, or a medical field, or equivalent experience preferred.
- Three or more years of experience working with seniors, disabled adults, human services, or general case management preferred.
- Must pass a criminal history background check.
- Must possess a valid driver's license and proof of current PPD test.
Working Environment
- Office-based position with minimal physical requirements (ability to lift 15 lbs., walking, and climbing stairs).
- Requires regular independent travel around the area served by Appalachian Agency for Senior Citizens.
- Some essential functions of the position can be performed remotely.
