Responsibilities
- Support clients through care transitions, promoting health and wellness
- Address social determinants of health and connect individuals with medical and community resources
- Assist clients in identifying personal health goals that promote independence
- Maintain accurate documentation within required timeframe
- Conduct outreach following hospital, emergency department, rehabilitation, or skilled nursing facility discharge
- Complete food insecurity and health-related social needs screenings
- Connect clients with Food is Medicine programs and nutrition resources
- Promote preventive care and routine follow-up with healthcare providers
Requirements
- Associates Degree with at least one year professional experience in the field or a Registered Medical Assistant with 2 years of experience
- Bilingual preferred
- Experience in primary care, hospitals, urgent care, rehabilitation, home health, skilled nursing, care coordination, or community health
- Experience working with individuals living with chronic medical conditions
- Excellent communication skills with the ability to establish and maintain positive and professional relationships with team members, consumers and their support systems, providers, and business partners
- Proficient in the use of computer systems, including Windows-based software applications such as Word, Excel, PowerPoint, and Outlook
- Effective documentation skills with the ability to complete documentation that is timely, accurate, and comprehensive
- Ability to process information accurately and make decisions based on specific situations and circumstances
- Ability to work both independently and as part of a team
- Strong work ethic, dependability, and commitment to consistently meeting responsibilities and deadlines
- Professional demeanor and ability to handle confidential information and documents with discretion according to GLSS policy and HIPAA guidelines and regulations
- Valid driver’s license and reliable transportation with the ability to conduct home visits, attend community events, or conduct other GLSS business throughout the GLSS service area
Pay
$22.50-$24.50 per hour
Schedule
Full Time, 40 hours per week (Monday-Friday). Hybrid, requires one-two days per week in the office. Community visits, home visits, and in-office meetings may be required as business needs dictate.
