Community Engagement Navigator (Mobile)

Responsibilities

• Deliver client services, including intake, service planning, life skills training, and coordination of other services to achieve client goals.
• Support and supervise brain injury survivors.
• Provide transportation and training on using transportation for brain injury survivors to meet their service needs.
• Document monthly progress towards identified goals.
• Maintain ongoing communications with program/agency staff and service providers.
• Provide crisis intervention and access emergency services to clients as needed.
• Identify and address community integration issues for clients on caseload or as designated.

Qualifications

• Bachelor's degree preferred with 1 year of relevant experience.
• Psychology, social work, special education, recreation therapy, communication sciences and disorders, sociology, kinesiology/exercise science graduates and pre-med gap year candidates encouraged to apply.
• Must be able to perform essential duties satisfactorily.
• Must have sensitivity to the needs of the population.
• Strong computer knowledge required.
• Strong analytical, numerical, and reasoning abilities.
• Excellent interpersonal skills and ability to work as part of a team.
• Ability to read English and communicate effectively in the primary language of the program.
• Valid drivers' license required.
• Operational and insured vehicle access required.
• Demonstrated understanding and competence in serving culturally diverse populations.
