The pay for this position is $44,850-$58,909 annually. $500.00 sign on bonus
Responsibilities
- Works directly with individuals receiving services
- Connects them with resources
- Documents client progress
- Supports follow-through on treatment plans and referrals
Requirements
- An Associate’s Degree and two years experience or Bachelor’s Degree
- The ability to establish and maintain supportive relationships with children, youth and families
- Knowledge on how to provide holistic and therapeutic case management services
- Experience working in the mental health field

- A short driving distance from Keene, Lebanon, and Hanover, New Hampshire
