Duties And Responsibilities
- Registers patients and obtains or verifies demographic, emergency contact, insurance, and other required information.
- Schedules patient appointments, testing, procedures, preventive services, and follow-up visits while ensuring accurate documentation within the electronic medical record.
- Performs patient outreach through approved communication methods regarding preventive care, referrals, follow-up needs, care gaps, quality initiatives, and patient engagement activities.
- Collaborates with providers, clinical staff, population health teams, and other departments to support quality improvement initiatives, patient satisfaction, care gap closure, and positive patient outcomes.
- Utilizes reports, quality metrics, and available data to identify, track, and complete patient care and population health activities.
- Collects patient payments and co-payments, accurately posts transactions, balances cash drawers when applicable, and comply with all PCI standards and organizational financial policies.
- Maintains department records, reports, files, and required documentation while ensuring accuracy and completeness of patient information.
Qualifications
- High school diploma or general education degree (GED).
- Must maintain BLS certification.
- One to three months related experience and/or training; equivalent combinations of education and experience may be considered.
- Experience in outpatient healthcare settings preferred.
- Population health, patient access, scheduling, or care coordination experience preferred.
- Ability to use hospital information systems, electronic medical records, Microsoft Office (Word, Excel, Outlook), and other healthcare software applications.
- Ability to adapt quickly to new computer systems and technology.
