About the role
The Community Support Specialist is a full-time, on-site role based in Lebanon, NH. This position provides direct support to individuals living with mental and behavioral health challenges, helping them access community resources, maintain stability, and build skills for independent living.
Responsibilities
- Coordinate services and assist individuals in accessing resources and maintaining independence.
- Support treatment and recovery plans.
- Facilitate connections to social, educational, and vocational opportunities.
- Advocate for client needs in collaboration with interdisciplinary teams.
- Regularly make home and community visits.
- Document services and progress.
- Ongoing communication with clients, families, and partner agencies.
Qualifications
- Demonstrated Community Support experience, including coordinating services and assisting individuals in accessing resources and maintaining independence.
- Strong Communication skills, including clear verbal and written communication, active listening, and the ability to build trusting relationships with clients, families, and colleagues.
- Background or education in Social Work, with knowledge of community-based services, case management, and client advocacy.
- Background or education in Psychology, with understanding of mental and behavioral health conditions and recovery-oriented, evidence-based practices.
- Experience working with individuals with Developmental Disabilities, including supporting life skills, inclusion, and person-centered planning.
- Additional beneficial qualifications: bachelor’s degree in social work, psychology, human services, or related field; familiarity with trauma-informed care; valid driver’s license and reliable transportation; ability to work collaboratively in multidisciplinary teams; basic computer and documentation skills.
