Community Based/Case Management RN

About the Role

ComForCare Home Care is a premier in-home care agency serving the Marion, Polk, and Linn/Benton county areas. We are seeking a self-motivated, friendly, and positive nurse who enjoys working with elderly and medically fragile clients. This part-time Community RN position offers flexible hours (10-20 hours per week), ideal for a semi-retired nurse who wants to remain active in their community without grueling hours. You will set your own appointments and manage your own caseload with minimal hands-on care, no after-hours call, and rare weekend requirements.

Responsibilities

• Conduct client assessments
• Perform delegations and teaching of nursing tasks
• Monitor and advocate for clients living at home

Requirements

• Licensed as a Registered Nurse in Oregon with a minimum of 3 years of experience
• 3+ years of experience teaching nursing tasks and performing delegations
• No restrictions on nursing practice
• Able to pass background and drug tests
• Current Oregon Driver’s License in good standing
