Responsibilities
- Provides direct patient care
- Provides education to patients/family on chronic disease processes
- Directly consults using in-depth clinical knowledge and skills in area of Diabetes to groups and individual patients with Type 1, Type 2, Gestational Diabetes and Pre Diabetes patient
- Assists in Diabetes Self-Management Education and Support (DSMES) program as well as Diabetes Prevention Program (DPP)
- Receives and assesses referrals from referring providers
- Consults with multidisciplinary team to develop, implement and evaluate care for patients and families
- Documents patient care activities in patient medical records
- Completes documentation accurately and timely
- Collaborates with appropriate disciplines to coordinate patient care
- Physician consultation for changes in patients' condition
Requirements
- Clinically competent registered nurse
- Graduate of an accredited school of nursing
- Currently licensed in the State of Ohio
- Two years of experience in delivering nursing care
- Current CDCES/CDE or willingness to obtain certification
- Analytical abilities and sound nursing knowledge
- Effective communication skills
- Knowledge of policies and procedures
- CPR certification
- Reliable transportation, current driver's license, and automobile insurance
Qualifications
