Case Manager, RN - Case Management
About the role
Pali Momi Medical Center, with 118 beds and a medical staff of over 400 physicians, serves the communities of Central and West O'ahu. The hospital is known for its innovative services such as an interventional cardiac catheterization unit, a minimally invasive surgical suite, a women’s center, and advanced imaging technologies. It is also recognized for its emergency room, which employs a team triage approach, and its case management team, which focuses on patient transitions and support.
Responsibilities
- Determine the appropriateness and medical necessity of admissions, continued hospital stay, and the use of ancillary services.
- Perform treatment plan assessments, discharge needs interviews, and planning/coordination for high-risk cases.
- Participate in organizational quality improvement activities.
Requirements
- Current Hawai’i Registered Nurse license.
- Bachelor's degree and/or equivalent combination of education, training, or work experience.
- Three (3) years clinical experience in acute care, community, or long-term care, or related field.

Skills
- Knowledge of Interqual.
- Case Management or Utilization Management certification preferred.
