Case Manager, RN - Case Management

About the role

Kapi'olani Medical Center for Women & Children is Hawai'i’s only maternity, newborn and pediatric specialty hospital. It is well recognized as Hawai'i’s leader in the care of women, infants and children. The Case Management team focuses on providing support and ensuring patients receive appropriate care and support tools.

Responsibilities

• Determine the appropriateness and medical necessity of admissions, continued hospital stay, and the use of ancillary services for unique and high-risk patient cases.
• Perform treatment plan assessments, discharge needs interviews, and planning/coordination for high-risk cases.
• Participate in organizational quality improvement activities.

Requirements

• Current Hawai‘i Registered Nurse license.
• Bachelor's degree and/or equivalent combination of education, training and experience.
• Three (3) years clinical experience in acute care, community or long term care, or related field.
