Community and Client Relations Specialist
Full-time: 32-40 hours/week
Reports to: Director of Missouri Slope at Home
About the role
The primary purpose of this position is to engage with prospective clients and their families to ensure they receive a comfortable home care experience with the quality care they deserve. The individual in this role will support our marketing strategies across various platforms, including developing digital content for social media and creating marketing collateral that aligns with Missouri Slope at Home’s brand identity.
Additionally, this role involves managing leads, completing contracts, and expanding home care services to drive organizational growth. The candidate will serve as the primary point of contact for clients and their families, effectively converting referrals into service admissions with a detailed service contract. A successful candidate will balance a consultative sales approach with community outreach and public relations by building referral networks, managing lead pipelines, and conducting community outreach activities.
Responsibilities
- Referral Source Development & Relationship Management
- Cultivate and maintain strategic relationships with referral sources, including hospital case managers, discharge planners, social workers, skilled nursing facilities, hospices, and assisted living directors.
- Conduct an established quota of face-to-face field sales visits and educational in-services weekly to ensure the agency remains top-of-mind.
- Educate partners on the continuum of care and how home care services (e.g., companion care, dementia support) allow seniors to age safely in place.
- Sales Process & Client Intake
- Manage the complete sales pipeline—from initial inquiry and lead generation through the booked assessment to the final Start of Care Services.
- Conduct pre-admission client evaluations and in-home care assessments, identifying the client’s physical and emotional needs.
- Collaborate internally with the Director, Nurse Manager, and Scheduling Coordinator as new clients are gained.
- Community Outreach & Brand Visibility
- Plan and host public relations events, seminars, and health fairs to boost brand awareness and directly reach seniors and their families.
- Actively engage with local senior centers, religious organizations, and community groups to provide educational resources.
- Performance Tracking & CRM Management
- Utilize updated agency CRM and scheduling software to log daily call activities, track referral sources, and monitor lead conversion rates.
- Provide weekly and monthly reports to franchise leadership, detailing activities, closed sales, and territory potential.
- Monitor market trends and competitor activities to identify new opportunities for growth and innovation.
- Client Case Oversight
- Ensure client contracts are complete and converted to service provision.
- Serve as point of contact for future client needs and service plan changes.
- Provide coordination and referral as needed to partner providers to ensure a well-rounded care plan, including services for Emergency Response Systems, Home health, Physical Therapy, Occupational Therapy, Transportation, and other necessary partners.
Requirements
- Must be at least 18 years of age.
- Associate degree in Marketing, Healthcare Administration, Business, Communications, Human Services, or a related field.
- Two to five years of experience in sales, healthcare marketing, admissions, case management, customer relations, or public relations preferred.
- Valid driver’s license with an acceptable driving record and reliable transportation.
- Ability to travel throughout the service area for community outreach, client visits, and networking activities.
- Proficiency with Microsoft Office Suite, Outlook, CRM software, social media platforms, and other business applications.
- Employees are required to receive the annual influenza vaccination as a condition of employment. Successful candidates must either get the vaccination (covered by the employer), provide proof of a current vaccination, or obtain an approved accommodation.

- Excellent verbal and writing skills, as well as strong organizational abilities.
- Capable of effectively communicating and working with a diverse range of individuals, including geriatric populations, people with special needs, and those requiring memory care support.
- Ability to work well with minimal supervision and effectively manage workflow.
- Ability to make independent decisions when necessary.
- Engage and recruit new residents by maintaining consistent leads in organizational software.
- Cultivate referrals through community outreach with healthcare professionals and other senior living organizations.
- Coordinate with internal teams to ensure a smooth transition to care.

- Regularly sit, stand, walk, use hands and fingers to operate office equipment and technology, reach with hands and arms, communicate effectively, and hear conversations in person and by telephone.

- Ability to safely perform all essential functions of a Personal Care Assistant (PCA)/Qualified Service Provider (QSP) when necessary.
