Responsibilities
- Provide strong support to a team of collaborative care coordinators, fostering a culture of collaboration, accountability, and continuous improvement.
- Demonstrate knowledge and skills in care coordination, ensuring seamless integration of healthcare services and resources.
- Create community partnerships that are dedicated to helping children and families overcome barriers as referral sources.
- Create quality care plans to optimize patient outcomes and enhance the quality of collaborative care.
- Maintain a collaborative care case load and provide coverage and back up to other team members.
- Conduct timely documentation, and monitor individual KPI’s.
- Ensure compliance with relevant regulations, standards, and best practices.
- Carefully coordinate care with state agencies, schools, behavioral health agencies, and community providers, to problem solve any issues and ensure effective resources are in place.
- Create referrals to community providers to support family needs.
- Provide customer service to families who indicate a need for care coordination and referral supports.
- Work with families in person or on the phone, being responsive to the family’s needs.
- Develop, recruit, organize, and facilitate group skills sessions or support groups, creating a safe and inclusive environment for participants to share experiences, build connections, and acquire essential skills.
- Evaluate the effectiveness of group interventions, gather feedback, and make data-driven adjustments to enhance the impact of the group sessions on individual and collective wellbeing.
Qualifications
- A minimum of 2 years of experience in case management or service coordination required.
- Experience and skill working with children and families who may be high risk or at risk.
- Knowledge of regulations and laws relating to clinical psychology/social work practices.
- Experience with behavioral health and mental health conditions.
- Strong interpersonal skills and ability to communicate effectively with families, colleagues (e.g. physicians, psychologists, social workers, and other team members), community agencies, etc.
- Understanding of principles of legal documentation and medical records release of information.
- Working knowledge of basic computer software programs: MS Office and EMR.
