About the Role

Perform personal care and homemaking services for clients in the community. Adhere to written and oral instructions prepared by primary nurse. Perform all aspects of personal care and hygiene to patients according to Hope Hospice Policy and Procedures. Utilize knowledge of proper and safe techniques related to ambulation, transfer, positioning, and range of motion (ROM), as well as Standard Precautions. Guide caregivers and families regarding the safe and comfortable personal care of the patient, including but not limited to activities of daily living (ADLs), Standard Precautions, mobility, and environment. Assist families of patients as needed. Document care and services, observations, and interventions for the primary nurse, managers, and interdisciplinary group (IDG).

Responsibilities

• Use proper and safe techniques for ambulation, transfer, positioning, and range of motion (ROM).
• Guide caregivers and families on safe and comfortable personal care, including ADLs, Standard Precautions, mobility, and environmental safety.
• Assist families of patients as needed.
• Document care, services, observations, and interventions for the primary nurse, managers, and IDG.
• Knowledge of meal preparation, feeding techniques, and related questioning.
• Flexibility to work in other locations if needed.

Requirements

• High school diploma or general education degree (GED).
• Certificate of completion of a qualified Certified Nurse Assistant (CNA) training program.
• Current Florida Certified Nursing License.
• Successful completion of a competency evaluation program prior to furnishing services to patients.
• CPR certification required.
• Valid Florida Driver’s License with a good driving record.
• Automobile insurance.
• Clear Level 2 background check.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Certification of completion of Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs.

Skills

• Excellent communication skills via phone, in person, and email.
• Good time management and organizational skills.
• Attention to detail and ability to handle confidential information following HIPAA guidelines.

Pay

Pay Range: $16.96 – $25.24 per hour.
