Registered Nurse - PACE Program

Role Overview

We are seeking a compassionate Registered Nurse to join a PACE team, providing holistic, participant-centered care to older adults.

Responsibilities

• Coordinate care within an interdisciplinary team to support independence and quality of life for elderly patients.
• Deliver strong direct patient care and care management in a community setting.
• Ensure compliance with BLS, First Aid, and other certification requirements.
• Maintain documentation and support patient well-being through personalized care plans.
• Collaborate with team members to optimize patient outcomes.

Qualifications & Skills

• BSN degree, RN license, and BLS certification.
• At least 1 year of experience working with the frail or elderly; 3+ years of nursing management in community-based care preferred.
• Basic Cardiac Life Support certification within 30 days of start.
• Strong communication, care coordination, and geriatric care skills.
• Commitment to compassionate, patient-centered care.
