About the role

Responsibilities

• Coordinating with an interdisciplinary team to deliver comprehensive geriatric care
• Providing strong direct patient care and care management
• Supporting community-based, home health services

Requirements

• BSN degree with RN license and BLS certification
• Minimum 1 year experience working with elderly or frail populations
• Basic Cardiac Life Support (must obtain within 30 days of hire)

Qualifications

Preferred 3+ years nursing management experience in community/home-based settings
