About the role

Provide in‑home care for PACE‑RI participants, delivering personal care (bathing, grooming, toileting, meals), light homemaking, behavioral support, and early detection of condition changes. Follow individualized care plans, document care in AxisCare, and collaborate with nurse oversight and rehabilitation‑department coaching for restorative activities. The position emphasizes relationship‑based work with the same participants each shift.

Responsibilities

• Assist participants with bathing, grooming, toileting, and meals.
• Perform light homemaking tasks.
• Provide behavioral support and monitor for changes in health status.
• Follow and document individualized care plans in AxisCare.
• Collaborate with nursing staff and receive training, including rehab‑department coaching for restorative activities.
• Maintain consistent relationships with assigned participants.

Requirements

• Active Rhode Island CNA license.
• Current CPR certification.
• High school diploma or equivalent.
• Valid driver’s license and reliable transportation.
• One year of CNA experience preferred (not required; PACE provides training).

Pay

$21–$24 per hour, based on PACE’s published postings and subject to confirmation with the employer.

Schedule

Day shift; weekend shifts may be available with additional bonuses.
