Covenant Health HomeCare and Hospice is the area’s largest locally owned home health and hospice provider, with one of the largest rehabilitation departments in East Tennessee. Responsibilities

  • Take and record temperature, pulse, respiration, blood pressure, and input-output measurements as outlined on the HHA Plan of Care (POC).
  • Provide morning or evening care, including bed bath, oral hygiene, shampooing and combing hair, back care, nail care, skin care, dressing the patient, changing bed linen, and cleaning the surrounding area.
  • Perform routine treatment procedures within the defined scope of practice for certified nursing assistants as outlined on the HHA POC.
  • Assist RN and/or LPN in treatments or procedures.
  • Provide general care such as positioning, lifting, or turning patients; assisting with bedpans, urinals, or commodes; aiding with ambulation and exercises; safe patient transfers including use of patient lifts; and application/removal of anti-embolism stockings.
  • Clean equipment and prepare for disinfecting or sterilization; report malfunctioning equipment.
  • Properly use equipment such as ice bags, ace bandages, and abdominal binders as outlined on the HHA POC.
  • Perform basic infection control techniques, including proper bag technique, hand washing, glove usage, appropriate waste handling, and keeping dirty and clean items separate.
  • Attend to nutritional needs of patients, including meal preparation and feeding.
  • Perform range-of-motion exercises under the supervision of Physical Therapy when appropriate.
  • Check schedules and call patients to provide arrival times.
  • Report any changes in patient condition or behavior and unusual incidents to licensed staff.
  • Assist in orienting new employees.
  • Demonstrate knowledge and competency in providing for age-specific needs of the population served.
  • Complete annual skills checklist and patient visit documentation.
  • Assist with medications that are ordinarily self-administered (including topical) as outlined on the HHA POC.
  • Local travel required.
  • Follow policies, procedures, and safety standards.
  • Complete required education assignments annually.
  • Work toward achieving goals and objectives and participate in quality improvement initiatives as requested.
  • Perform other duties as assigned.

Requirements

  • High school diploma or equivalent.
  • Six (6) months of hospital or related experience.
  • CNA licensure from the State of Tennessee required upon hire. Out-of-state hires must have the Tennessee CNA license “in process” as reflected on the state website upon hire.
  • CPR certification required.
  • Valid Tennessee driver’s license (Class D) and state-mandated minimum insurance coverage.
  • Driving record must meet Covenant Health minimum standards at the date of hire and throughout employment.
