About the Role
Provide routine daily nursing care and services to assigned residents in accordance with their assessment and care plan, under the direction of supervisors.
Responsibilities
- Identify residents using wristbands or photo cards before administering treatments or serving meals.
- Use only authorized abbreviations when recording information.
- Report all changes in resident’s condition to the Nurse Supervisor/Charge Nurse promptly.
- Record entries on flow sheets, notes, and charts in an informative and descriptive manner.
- Report all accidents and incidents observed during the shift.
- Protect resident’s protected health information and report any suspected violations.
- Report unauthorized attempts to access the facility’s information system.
- Perform only nursing care procedures for which you have been trained.
- Prepare resident’s room for their arrival (e.g., bed made, name tags, admission kit available).
- Greet residents, escort them to their room, and introduce them to roommates and staff.
- Make residents comfortable, inventory and mark personal possessions, and store clothing.
- Assist residents with packing personal possessions for room transfers or discharges.
- Transport residents to new rooms or receiving areas and assist with loading/unloading from vehicles.
- Maintain a warm, calm, and positive environment for residents.
- Meet regularly with nursing personnel to identify and correct problem areas or improve services.
- Report all resident complaints and grievances.
- Participate in and receive nursing reports as instructed.
- Follow established policies for exposure to blood/body fluids.
- Make beds, provide extra covers, and measure and record vital signs (e.g., temperatures, pulses, respirations) as instructed.
- Weigh and measure residents as instructed.
- Frequently check on residents unable to call for help.
- Answer resident calls promptly and ensure personal care needs are met.
- Assist residents with food arrangements and record food/fluid intake.
- Report changes in eating habits to the Nurse Supervisor/Charge Nurse.
- Keep water pitchers clean, filled, and within reach of residents.
- Perform after-meal care (e.g., remove trays, clean residents).
- Perform all assigned tasks according to established policies and procedures.
- Follow work assignments and schedules to complete tasks.
- Cooperate with inter-departmental personnel to maintain nursing services.
- Notify the facility of lateness or absence from work.
- Report occupational exposures to blood, body fluids, or hazardous chemicals.
- Assist residents with daily functions (e.g., dental care, bathing, dressing, combing hair).
- Keep residents dry by changing wet or soiled gowns, clothing, and linens.
- Assist residents in preparing for medical tests (e.g., lab work, x-rays, therapy).
- Assist with lifting, turning, moving, positioning, and transporting residents.
- Perform restorative and rehabilitative procedures as instructed.
- Prepare residents for physical examinations.
- Check residents routinely to ensure personal care needs are met.
- Observe and report pressure areas, skin breakdowns, or injuries of unknown source.
- Observe disoriented and comatose residents and record/report data as instructed.
- Perform special treatments as instructed.
- Report any resident leaving or missing from the facility immediately.
- Use only equipment you have been trained to operate and report defective equipment.
- Maintain confidentiality of all resident care information, including protected health information.
- Report allegations of resident abuse or misappropriation of property.
- Honor resident’s refusal of treatment and report such requests to supervisors.
Requirements
- Minimum 10th grade education.
- Ability to read, analyze, and interpret professional journals, technical procedures, or regulations.
- Ability to write reports, business correspondence, and procedure manuals.
- Ability to present information and respond to questions from managers and employees.
- Ability to apply mathematical concepts (fractions, percentages, ratios) to practical situations.
- Ability to solve practical problems with limited standardization.
- Ability to interpret instructions in written, oral, diagram, or schedule form.
- Licensed Certified Nursing Assistant in accordance with state laws.
- CPR certification preferred.
Physical Demands
- Occasionally lift and/or move up to 25 pounds.
- Prolonged use of desktop or laptop computer.
- Regularly required to sit, stand, walk, talk, read, or hear.
- Frequent use of office equipment (e.g., copier, scanner, fax, telephone, calculator).
- May assist in evacuating residents during emergencies.
Work Environment
The work environment is usually low to moderate noise. Reasonable accommodations may be made for individuals with disabilities.
