Requirements

  • Active South Carolina CNA Certification (from an accredited program)
  • CPR Certification
  • Long-term care experience preferred

Responsibilities

  • Provide each assigned resident with routine daily nursing care and services in accordance with the resident’s assessment and care plan, as directed by supervisors.
  • Identify residents using wristband or photo card file before administering treatments or serving meals.
  • Report all changes in the resident’s condition to the Nurse Supervisor/Charge Nurse.
  • Record all entries on flow sheets, notes, or charts in an informative, descriptive manner.
  • Report all accidents, incidents, and changes in resident condition observed during the shift.
  • Ensure resident rooms are prepared for new admissions (e.g., bed made, name tags up, admission kit available).
  • Greet residents, escort them to their rooms, and introduce them to roommates and staff.
  • Inventory and mark residents’ personal possessions; assist with packing/unpacking during transfers or discharges.
  • Transport residents to new rooms or receiving areas; assist with loading/unloading residents to/from vehicles.
  • Maintain a warm, personal, and calm environment for residents.
  • Participate in shift reports and nursing meetings to identify and address problem areas.
  • Report all resident complaints and grievances.
  • Follow established policies for exposure to blood/body fluids.
  • Perform routine tasks such as making beds, measuring vital signs (TPRs), and weighing residents.
  • Assist residents with daily functions (e.g., dental/mouth care, bathing, dressing, grooming).
  • Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
  • Assist with lifting, turning, moving, positioning, and transporting residents using proper techniques.
  • Perform restorative and rehabilitative procedures as instructed.
  • Observe and report pressure areas, skin breakdowns, or injuries of unknown source.
  • Monitor disoriented or comatose residents and report observations as instructed.
  • Perform special treatments as instructed and report any resident leaving or missing from the facility immediately.
  • Use only equipment you are trained to operate; report defective equipment.
  • Maintain confidentiality of all resident care information, including protected health information.
  • Report suspected violations of unauthorized disclosure of resident information.
  • Report allegations of resident abuse or misappropriation of resident property.
  • Honor resident’s refusal of treatment and report such requests to your supervisor.

Qualifications

  • Minimum 10th grade education.
  • Ability to read, analyze, and interpret professional journals, technical procedures, or regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to apply mathematical concepts (fractions, percentages, ratios) to practical situations.
  • Ability to solve practical problems and interpret instructions in written, oral, or diagram form.
  • Licensed Certified Nursing Assistant in accordance with South Carolina state laws.
  • CPR certification preferred.

Physical Demands

  • Occasionally lift and/or move up to 25 pounds.
  • Prolonged use of desktop or laptop computer.
  • Regularly required to sit, stand, walk, talk, read, or hear.
  • Frequent use of office equipment (copier, scanner, fax, telephone, calculator).
  • May assist in evacuating residents during emergencies.

Work Environment

  • Noise level is usually low to moderate.
