- Holland Home Skilled Nursing: Full time 1st shift bath aide; Part time 2nd; Part time 3rd
- Atrio Home Care: Full Time Days - home visits - Lakeshore & Grand Rapids
- Atrio Help At Home: Per Diem all shifts - home visits - Lakeshore & Grand Rapids
- Care Resources PACE: Full Time Days - home visits
- Faith Hospice: Full time 1st shift - home visits

Responsibilities
- Provide direct patient care to residents in community or home settings, including basic life support, hygiene, mobility assistance, and daily living activities
- Monitor patient condition, report changes, and document in EHR systems accurately
- Assist with feeding, medication reminders, and coordination with nursing staff and supervisors
- Ensure safety through proper body mechanics, fall prevention, and adherence to care plans
- Support geriatric and memory care needs, including Alzheimer's and long-term care considerations
Qualifications
- CNA with certification and current license
- Basic life support, patient care, and manual handling skills
- Experience in home/community care and geriatric settings
- Proficiency with EHR systems
- Knowledge of physiology and nursing home care; ability to support memory care and senior patients
- Strong patient service orientation and ability to work in long-term or post-acute care environments
