• Holland Home Skilled Nursing: Full time 1st shift bath aide; Part time 2nd; Part time 3rd
• Atrio Home Care: Full Time Days - home visits - Lakeshore & Grand Rapids
• Atrio Help At Home: Per Diem all shifts - home visits - Lakeshore & Grand Rapids
• Care Resources PACE: Full Time Days - home visits
• Faith Hospice: Full time 1st shift - home visits

Responsibilities

• Provide direct patient care to residents in community or home settings, including basic life support, hygiene, mobility assistance, and daily living activities
• Monitor patient condition, report changes, and document in EHR systems accurately
• Assist with feeding, medication reminders, and coordination with nursing staff and supervisors
• Ensure safety through proper body mechanics, fall prevention, and adherence to care plans
• Support geriatric and memory care needs, including Alzheimer's and long-term care considerations

Qualifications

• CNA with certification and current license
• Basic life support, patient care, and manual handling skills
• Experience in home/community care and geriatric settings
• Proficiency with EHR systems
• Knowledge of physiology and nursing home care; ability to support memory care and senior patients
• Strong patient service orientation and ability to work in long-term or post-acute care environments
