About the role

Ravenwood Health is hiring full-time Care Coordinators in Eastern Cuyahoga, Lake, and Ashtabula Counties. Care Management Entity (CME) is a program in collaboration with Aetna OhioRISE designed to help young people (0-20) with behavioral health challenges.

Responsibilities

  • Regularly engage with youth and families on a community-based level in a culturally competent manner.
  • Assess level of care using the CANS assessment.
  • Connect youth and families/caregivers to resources and natural support and work collaboratively with wraparound teams.
  • Organize wraparound team meetings and maintain documentation of these meetings.
  • Link youth and families to specialized clinical resources to meet their treatment needs.
  • Provides assistance and support in crisis situations; responds effectively and mobilizes access to resources in crisis situations.
  • Works closely with youth and families to develop care plans that incorporate their voice and choice, and regularly monitors and adjusts care plans as necessary.
  • Collects outcome information by administering and collecting necessary forms to client and family.

Requirements

  • A minimum of three years’ experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field, providing community-based services to children and youth and their family or caregivers.
  • Must have a valid driver’s license.
  • Must pass a background check.

Education and Experience
