Care Coordinator

About the role

Operation PAR, Inc. is a non-profit organization offering comprehensive behavioral health treatment in west Florida. PAR has been providing services since 1970 through advocacy, education, clinical services, and research.

Responsibilities

• Manage referrals for PAR’s services (phone lines, CAM, external entities, and internal referrals)
• Screens clients for admission into services, ensuring they meet necessary criteria for substance abuse and mental health treatment
• Conduct initial assessments, review treatment consents, and explain patient rights and responsibilities
• Manage the intake process, including financial reviews and documentation
• Use telehealth services for clinical screenings and counseling
• Perform various administrative tasks as assigned
• Care Coordination for clients, including intake, discharge planning, outreach, and linkages to other services both within and outside PAR

Requirements

• Understanding the complex nature of clients: Knowledge of substance use, mental health, harm reduction, and trauma informed care
• Identify effects of psychoactive substances on clients and families
• Understand co-existing medical and psychological disorders
• Treatment Knowledge: Know accepted levels of treatment, recovery, and relapse prevention
• Participate in interdisciplinary treatment planning
• Knowledge of community and internal resources
• Meet contractual and program service standards
• Documentation: Timely documentation of all client information and screenings in the agency's electronic health record (EHR) system
• Adhere to state, federal, and agency regulations regarding client confidentiality and service provision
• Maintain accurate records for auditing and quality assurance purposes

Qualifications

• High School Diploma plus two years of direct client service experience in mental health or substance abuse
• Bachelor’s degree in a directly related field plus two years of direct client service experience in mental health or substance abuse
• Master’s degree in counseling or a directly related field with two years of direct client service experience in mental health or substance abuse, licensed or pursuing licensure preferred
• MCAP, CAP, or CAC certification from the Florida Certification Board preferred

Education and Experience

• Must meet one of the following criteria:
• High School Diploma plus two years of direct client service experience in mental health or substance abuse
• Bachelor’s degree in a directly related field plus two years of direct client service experience in mental health or substance abuse
• Master’s degree in counseling or a directly related field with two years of direct client service experience in mental health or substance abuse, licensed or pursuing licensure preferred

Essential Physical/Technical Skills

• Ability to move about treatment facility, both indoors and out
• Ability to lift and carry 5 to 10 pounds
• Proficiency with Microsoft Windows-based computers
• Ability to document in an electronic health record system
• Ability to use Zoom and Teams virtual platforms
• Highly organized
• Familiarity with Data Collection and Analysis

• Adheres to company time and attendance standards, with understanding of the needs of a 24-hour residential program if applicable
• Demonstrates knowledge of company policy and procedure

Special Requirements

• Must have a valid driver's license
• Must pass a level II background check (https://info.flclearinghouse.com) and drug screen
• Must maintain current training as required by Operation PAR, Inc. policies
