Care Coordinator

Requirements

• The ideal candidate will possess the following qualifications:
  • A Bachelor’s degree in a social services related field (social, psychologist, human services, etc)
  • Master’s degree preferred
  • A minimum of three years experience in adult mental health care
  • Strong customer service and de-escalation skills
  • Demonstrated ability to work cooperatively in a Recovery-Based team environment

Responsibilities

• Serves as a floor leader within the urgent care setting, overseeing patient flow to ensure individuals move efficiently through the clinic’s workflow and crisis response protocols.
• Coordinates the initial engagement and triage processes, while ensuring all required information is accurately documented in the electronic health record (EHR).
• Promotes a person-centered experience aligned with a “living room” model of care, ensuring individuals feel supported and informed while awaiting urgent services.

Pay

The hourly rate is $29/hr. as it includes a $2/hr shift differential!

Schedule

This posting is Full-time, First Shift (Monday - Friday 10:30am - 7:00pm EST).
