CLINICIAN II/III, INTENSIVE CARE COORDINATOR, CHILD AND FAMILY- CHARLOTTESVILLE (FULL-TIME)

About the role

This is a FLSA non-exempt position. Seeking an individual to join our collaborative team and lead child and family specific, wraparound teams in the High Fidelity Wraparound Model. The incumbent will support youth and families through team facilitation, clinical engagement, assessment, service planning, linking to services, monitoring service delivery, crisis management, conflict resolution, collaboration with community partners and service providers, and group process management skills.

Responsibilities

• *Completes comprehensive strengths based child and family assessments.
• *Conducting diagnostic assessments, including integration of observational data, in determining service eligibility and referral needs.
• *Completing program specific needs assessments and authorizations necessary for Medicaid or CSA reimbursement.
• *Assessing needs and planning services to include developing a comprehensive individualized services plan.
• *Facilitate wrap around teams.
• *Enhancing community integration through increased opportunities for community access and involvement with natural supports.
• *Linking monitoring service delivery through contacts with individuals receiving services, service providers and periodic site and home visits to assess the quality of care and satisfaction of the individual.
• *Providing clinical review of services for identified CSA service recipients to assure quality of care & collaborate with case agency to review and follow up on funding.
• *Reading all agency communication (i.e., Ten, Region Ten’s Newsletter, e-mail, etc.).
• *Providing individual and group counseling, as approved by supervisor, to meet licensure core competencies.
• *Other duties as assigned by the supervisor in keeping with the general requirements of the position.

Qualifications

• CLINICIAN II: Master’s degree and license eligible according to the DHP definition. Education verified by an official transcript with conferral date. Evidence you have had an internship or practicum of at least 500 hours of experience with persons who have a mental illness. (Verification of internship/practicum for QMHP form).
• CLINICIAN III: Licensed clinicians as one of the following: Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Clinical Psychologists.

Pay

HOURLY SALARY RANGE:
GRADE 10- CLINICIAN II: $25.00 - $34.68 per hour
GRADE 11- CLINICIAN III: $27.40 - 38.15 per hour

Schedule

Full Time (Non-Exempt); Monday-Friday 8:30-5:00pm
