Position Overview

The Clinical Transplant Coordinator fulfills the primary responsibilities of coordinating the multidisciplinary approach in the evaluation, treatment, and facilitation of the transplant process, including follow-up care for organ recipients and donors. The role emphasizes collaboration, coordination, and consultation with nursing leadership, primary nurses, and other healthcare team members to ensure seamless transplant care.

Key Responsibilities

  • Cook up multidisciplinary efforts to coordinate evaluation, treatment, and follow-up care for transplant patients and donors.
  • Collaborate with nursing leadership, primary nurses, and healthcare team members to facilitate smooth transplant processes.
  • Manage multiple tasks and projects related to transplant care, ensuring timely and effective service delivery.
  • Track patient care activities, document outcomes, and support quality improvement initiatives.
  • Educate and support patients and families throughout the transplant process.

Qualifications & Requirements

  • Education: Associates of Nursing (ASN) required. Bachelor of Nursing (BSN) preferred. ASN RNs hired after January 1, 2013, must complete their BSN within five (5) years of hire.
  • Licensure & Certifications: Active Registered Nurse (RN) license in Indiana or Nurse Licensure Compact (NLC) RN license. Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • Experience & Skills: Demonstrated clinical competency and leadership skills. Clinical practice experience related to transplant programs. Strong organizational, writing, interpersonal, and verbal communication skills. Critical thinking skills and ability to work independently. Ability to manage multiple tasks and projects effectively.
