Clinical Support RN - (Full-Time 1.0/Days)

RN – Clinical Support. In-office role, 40 hrs. per week. Shift option: 8am – 5pm or 9am – 6pm. No holidays/weekends.

Functions

• Triages calls assessing pain and symptoms, physical needs and disease progression
• Provides direction to the caller or coordinates necessary follow-up, prioritizing for those with uncontrolled pain/symptoms and imminent death
• Provides anticipatory guidance and education for disease progression and end of life care. Utilizes patient teaching tools and handbooks as reference.
• Assists with telephonic assessment and coordination of inpatient center transfers
• Obtains refills, renewals, new and revised orders for medication and treatments
• Supports the initiation of symptom management orders through timely notification to contracted pharmacy
• Updates the clinical record with new orders, medications and treatments, and communicates to team.
• Anticipates medication, DME and supply needs and bundles orders.
• Serves as a resource and mentor to hospice clinical staff

Qualifications

• Registered Nurse licensed in Pennsylvania
• Associates in nursing required
• Minimum of two years hospice, medical/surgical, home health, or relevant experience required
• Must be able to complete 5–6 weeks of a FT (40 hr./wk.) orientation.

Must work a minimum of 64 hours per two-week pay period for starred benefits. Must work a minimum of 40 hours per two-week pay period for caret-marked benefits.
