About the role

Responsibilities
- Provide services in residential, vocational, or in-home settings, supporting individuals with disabilities.
- Ensure support plans and services adhere to Individual Plans, quality standards, and regulatory requirements.
- Complete structured assessment interviews with parents/care providers and conduct observations in various settings.
- Review social history information, complete functional assessments, and gather baseline information for behavior support plans.
- Write and submit summaries of assessments and behavior support plan components to interdisciplinary teams or operations leaders.
- Advocate for the human and civil rights of individuals by presenting behavior support plan information to review committees.
- Document progress, review records, and maintain confidentiality while completing billing documentation as necessary.
- Maintain healthy and professional relationships with individuals, families, guardians, and case managers.
- Implement the company’s Customer Service Standards and report any instances of alleged abuse or neglect according to internal and external standards.
- Support and train staff in implementing Individual Support Plans and conduct classes and orientations as assigned.
- Attend staff meetings and interdisciplinary team meetings as needed.
- Assist with socialization, behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living as needed.
- If assigned, accurately administer and document the delivery of medications and treatments, promptly reporting any errors.
- If assigned, monitor individual health, document concerns, and communicate with nurses or supervisors as appropriate.
- May transport individuals into the community, ensuring safe driving practices and reporting accidents and safety concerns.
- Participate in safety drills and protect individuals during emergencies.
- Comply with all established safety policies, procedures, and rules.
Qualifications
- Bachelor’s Degree in a human services field.
- Two years of related experience.
- Training in behavior modification techniques and/or experience providing behavior management treatment as required by state or program funder.
- Knowledge of specialized populations such as individuals with intellectual disabilities, brain injuries, or mental health issues.
- Expertise in special disciplines like behavioral support, early intervention, or crisis intervention.
- A reliable, responsible, and caring nature with the ability to work well with others.
- A commitment to the company’s mission and values.
- A current driver’s license, car registration, and auto insurance if providing transportation for individuals receiving services.
- All state-required training and certification completed in mandated timeframes.
