About the role

The Clinical Social Worker, MSW at PIH Health interviews and evaluates patients and family members to identify social, emotional and economic factors which may interfere with obtaining maximum benefits from medical care. They develop and implement a social work treatment plan, select and utilize community resources, make referrals to and coordinate with other professional disciplines or agencies, provide psycho-social therapy, participate in multidisciplinary health team conferences, and assist in the coordination of the discharge plan.

Responsibilities

  • Interviews and evaluates patients and family members to identify social, emotional and economic factors affecting medical care.
  • Develops and implements a social work treatment plan.
  • Selects and utilizes community resources as part of the treatment plan.
  • Makes referrals to and coordinates with other professional disciplines or agencies.
  • Provides psycho-social therapy using a variety of treatment methods.
  • Participates in multidisciplinary health team conferences to formulate the overall patient care plans.
  • Aids in the coordination of the discharge plan.

Requirements

  • Demonstrates skill in patient assessment, intervention, planning, and evaluation.
  • Demonstrates initiative, good judgment, and the ability to make decisions independently.
  • Demonstrates skill in working with patients having planned and unplanned life transitions such as but not limited to diagnosis of chronic and acute illness, death and dying, and complex family dynamics.
  • Strong communication skills, verbal, non-verbal and written.
  • Ability to learn department and hospital policies and protocols.
  • Knowledge of managed care payment requirements and Medi-Cal and Medicare requirements.

Qualifications

  • Master’s Degree from an Accredited School of Social Work.
  • Current BLS from the American Heart Association (if assigned to Palliative Care, Hospice or Home Health).
  • Evidence of continuing education.
  • Management will track clinical hours.
  • Experience with regulatory requirements related to reporting of child/elder abuse, domestic violence, and other legal requirements.
  • Must possess valid, unrestricted California Driver’s License and valid auto liability insurance.
