Responsibilities
- Manage a caseload of 15-20 patients daily
- Cook up discharge plans collaboratively with a team
- Work in a team environment to ensure efficient patient care
Requirements
- Minimum of 2 years of acute inpatient case management experience, including at least 1 year in a large hospital (500+ beds, Level 1)
- Proficiency with Epic EMR and strong computer skills
Qualifications
- Active LMSW or LCSW license
- BLS certification
- Recent inpatient case management experience
Skills
- Effective communication and collaboration skills
- Strong organizational and time management skills
- Ability to work efficiently with minimal training
